To register your student, the following documents are needed on the appointment day:

• Up-To-Date Health Records

• Copy of Birth Certificate

• Proof of Address - Students enrolling, must provide address verification by presenting TWO original documents to the school secretary to complete the enrollment process (a driver license and a bill or document with parent current address listed)

• Enrollment Forms

• A physical is required for all incoming kindergarten students
